    You may order up to 10 copies of the free pamphlet at no cost. If you order more than 10 copies of the same pamphlet you will be charged $0.30 per pamphlet over and above 10.

    How do I order?

    • This is an online order form – just put in the number of publications you want in the corresponding box, add your name, address and phone number,  we will confirm the order with you by phone we will mail you the documents you requested. 
    • There is a postage and handling fee (=15% of total order) There is a $2.00 minimum
    • Payment (for quanties over 10) is required before orders are shipped. Please allow 4 weeks for delivery.
